When to go because soreness changed
The joint may wake you at night or stop you halfway up the stairs. It may swell after an ordinary outing and stay stiff the next morning. When that keeps happening, an exam can help find the reason. You can't tell from soreness alone whether wear or an old injury is involved.
Notice whether the joint holds your weight and moves as usual. Don't leave out a fall, twist, swelling, or fever. Your clinician can use those details to decide how soon to see you. If you told me the change was sudden or severe, I'd call now.
When to get help without waiting
Fever along with warmth and heavy swelling needs quick medical care. So does a joint that looks bent after a fall or won't hold your weight. Sudden weakness, numbness, or lost bladder control can mean a nerve emergency, not ordinary joint wear. Don't wait for a planned clinic appointment.
New swelling after any joint procedure also needs quick attention. Tell the office when it began and whether you have chills. If you can't reach the office, go to urgent care. More treatment can't come before an exam.
What to bring for a planned visit
Take prior X-ray reports and list every medicine you use. Add a brief record of what causes soreness and what helps. Tell the clinician about physical therapy, braces, or earlier care. You don't need details that have nothing to do with this joint.
Ask what the exam found and what you can still try at home. If a procedure comes up, ask about driving, time off, follow-up, and total cost. The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

A four-arm, multicentre, single-blind phase 2/3 randomized trial of 480 knee OA patients (KL II-IV) compared autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction and allogeneic umbilical-cord-tissue mesenchymal stromal cells against a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another, or to the corticosteroid control, and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events occurred.

A 2026 systematic review and meta-analysis of 28 randomized trials of intra-articular mesenchymal stem cell-based therapies in knee OA found significant improvements in several pain and function measures (delta-VAS MD -1.67; KOOS pain MD 15.37) but NO significant difference in WOMAC, KOOS quality of life or the Lequesne index, and MRI-based WORMS scores were non-significant - indicating no consistent structural benefit. Its own conclusion: these therapies serve a primarily SYMPTOM-modifying rather than STRUCTURE-modifying role, with higher frequencies of local reactions to weigh against the symptomatic benefit.

The SUMMIT randomized trial treated 144 patients (mean age 33.8, mean lesion 4.8 cm2) with at least one symptomatic focal cartilage defect (Outerbridge III/IV, >=3 cm2) of the femoral condyle or trochlea. Matrix-applied characterized autologous cultured chondrocytes (MACI) improved KOOS pain (37.0 to 82.5) and function significantly more than microfracture (pain 35.5 to 70.9) at 2 years, with histological and MRI assessment of the repair tissue. This is what a positive cartilage-repair trial looks like - in young patients with a discrete hole, not a worn joint.

A randomized trial in the New England Journal of Medicine compared physical therapy against intra-articular glucocorticoid injection for knee osteoarthritis and found physical therapy produced better WOMAC outcomes at one year. When a clinic offers an injection, the comparator that matters is not 'nothing' - it is a course of supervised exercise.

What to bring to the first visit
Bring a medicine list, earlier joint reports, and notes about what worsens the soreness. Licensed clinic staff will examine the joint. They'll explain whether a blood-based regenerative treatment may fit and what the visit would involve.
