What to notice when you first stand

Before you rise, the joint can already feel tight. A knee can feel full, while a shoulder may catch as you dress. The ache might ease with breakfast and return during errands. If you asked where to begin, I'd ask when the soreness changed.

Years of use, an old injury, weak muscles, or added strain can cause soreness. Sitting can make an arthritic joint feel stiff. Wear can appear on your X-ray without showing how much you hurt. Tell your clinician which chores, walks, or hours of sleep have changed.

What to try on a sore day

Ease the activity that stirred the joint, but don't stay still all day. Cold may settle swelling, although it won't suit everyone. Warmth before movement can ease stiffness. A long outing may hurt more than several brief walks.

Test only warmth or only cold for several days. Don't alter your usual walking and chores during that test. Write down what you tried and how the joint felt later. If soreness rises sharply, stop that activity and ease back.

When to have the joint examined

Book an exam if soreness keeps returning, interrupts sleep, or changes your walk. You'll be asked about old injuries and the care you've already tried. The clinician will move and press the joint, then review any X-ray. Ask what's causing the soreness and what can come before surgery.

Get prompt care when major swelling, heat, fever, or lost movement starts quickly. A new injury or sudden weakness can't wait either. The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

FDA states plainly that no stem cell, exosome, stromal vascular fraction, umbilical cord blood, Wharton's jelly or amniotic-fluid product has been approved for the treatment of ANY orthopedic condition - it names osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain and shoulder pain individually. The only FDA-approved stem cell products in the United States are cord-blood-derived blood-forming stem cells for disorders of the hematopoietic system, and there are currently no FDA-approved exosome products.
