No significant difference in clinical scores emerged at long-term follow-up; there were 17 failures in the ACI group versus 13 in the microfracture group and more total knee replacements after ACI (6 versus 3). Fifty-seven percent of surviving ACI patients and 48% of surviving microfracture patients had radiographic early osteoarthritis (KL >=2).

The SUMMIT randomized trial treated 144 patients (mean age 33.8, mean lesion 4.8 cm2) with at least one symptomatic focal cartilage defect (Outerbridge III/IV, >=3 cm2) of the femoral condyle or trochlea. Matrix-applied characterized autologous cultured chondrocytes (MACI) improved KOOS pain (37.0 to 82.5) and function significantly more than microfracture (pain 35.5 to 70.9) at 2 years, with histological and MRI assessment of the repair tissue. This is what a positive cartilage-repair trial looks like - in young patients with a discrete hole, not a worn joint.

The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

FDA states plainly that no stem cell, exosome, stromal vascular fraction, umbilical cord blood, Wharton's jelly or amniotic-fluid product has been approved for the treatment of ANY orthopedic condition - it names osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain and shoulder pain individually. The only FDA-approved stem cell products in the United States are cord-blood-derived blood-forming stem cells for disorders of the hematopoietic system, and there are currently no FDA-approved exosome products.

Notably it does NOT strongly recommend any biologic injectable.

## Why is my joint stiff in the morning?

A worn or irritated joint often stiffens while you rest. It may loosen after a few minutes and ache again after activity. Note how long stiffness lasts and whether swelling comes with it. That'll help your clinician understand the day-to-day problem.

## Can cartilage regenerate after years of wear?

Cartilage regeneration across a widely worn joint hasn't been shown through exercise or a clinic procedure. That doesn't mean exercise is wasted. It can make the joint stronger and easier to use. Ask whether the X-ray shows one damaged area or widespread wear.

## What can I try at home for soreness?

Give the joint a lighter day without sitting through all of it. Try short walks, warmth before movement, or cold after activity. A well-fitted brace or cane may also help. Ask your doctor or nurse what's safe with your health.

## Which warning signs need fast care?

Get prompt help when fever comes with a hot or badly swollen joint. Don't delay after a fall if the joint bends oddly or can't hold weight. Sudden weakness or numbness matters too. These problems need an exam before a planned clinic appointment.

## What is PRP?

PRP means platelet-rich plasma: a clinic takes some blood and separates a portion with more platelets. The prepared blood is placed directly in the joint that hurts. Clinics don't all prepare it alike, so ask exactly what's offered. It may ease soreness, but it can't promise new cartilage.

## What is regenerative medicine?

The term regenerative medicine covers joint procedures using blood, bone marrow, fat, or donated birth tissue. The name doesn't mean that new cartilage will grow. Ask what will be used and what improvement is expected. Calmer soreness and new cartilage are different results.

## How much time and money might care take?

Time and cost depend on the treatment and the clinic. If you asked me what to get in writing, I'd request the entire price and planned visits. Ask about time away from driving or work. Get the follow-up date in writing too.
